Champaign County Regional Planning Commission’s (CCRPC) Disability Services team offers Case Management services to individuals with I/DD in Champaign County. Our team is available for person-centered planning, dual diagnosis, and transition consultant needs. Individuals must be registered on PUNS to qualify for services.
