Avicenna Community Health Center is a free, primary care clinic available for 18 and over uninsured and underinsured residents of Champaign County. The clinic can manage diabetes, high blood pressure, and high cholesterol. Other services include physical exams, nutritional counseling, social work services, prescription assistance, and management of other primary health conditions. We are open Sundays from 1 – 4pm for scheduled appointments and walk-in visits. Appointments can be scheduled at 217-418-5888. Leave a message and we will call you back between 5:30 and 9:30pm on Wednesdays and Thursdays. Spanish, Mandarin, Arabic, and French language interpreters are available with advanced notice.

The Carle Inpatient Rehab Unit treats hospitalized patients who are medically stable and able to participate in three hours of therapy a day in order to maximize independence and return home. Therapy Services provided at Carle includes Physical, Occupation and Speech Therapy in outpatient and inpatient settings.  Specialties at the hospital location include: Women’s Health, Community Reentry, Lymphedema and Orthopedics. We are pleased to also be able to share The Brain Injury Association of Illinois (www.biail.org) resources. Carle hosts a monthly brain injury support group through the BIA of IL. Please call to confirm meeting dates and times. Meetings are held at the Carle – Mills Cancer Center – 2nd Floor Houseworth Conference Room.

CCHCC provides free information, assistance, referral, and advocacy services to community members who are having problems with any aspect of the health care system. CCHCC’s Consumer Health Hotline helps consumers find low-cost health care resources, understand their patient rights, find discount prescription programs, apply for Medicaid/AllKids, apply for financial assistance at local hospitals, and more. Health Care Consumers also operates the Dental Referral Program, a program that connects low-income, uninsured consumers with affordable dental care. The Medicare 100/Plus Program helps Medicare beneficiaries get discounts on their out-of-pocket costs at Presence Covenant Medical Center and at some local independent doctors.

The County Health Department funds, supports, and directs the activities of the Champaign County Child Dental Access Program, which serves the oral health needs of children living in Champaign County outside Champaign-Urbana.  All other services are provided via a contractual relationship with the Champaign-Urbana Public Health District.

The Champaign-Urbana Public Health District offers an array of services and programs that reach people of all ages and from all walks of life. Pregnancy care, nutrition advice, educational support, supplemental food coupons, pregnancy tests, genetics referrals and breastfeeding support. People living with HIV and AIDS rely on us for case management, flu, pneumonia shots and other immunizations. We inspect food and drink establishments, and tanning facilities. We conduct lead investigations, license wells and provide food-handling instruction. We investigate disease outbreaks that could threaten community health, and we are part of a county-wide emergency preparedness team.

Crosspoint Human Services provides 24-hour intensive service for children and youth in acute mental health crisis who may need hospitalization. Crosspoint also provides Outpatient Therapy and Counseling Services by licensed and Qualified Mental Health professionals. Counseling and Therapy services include individual and family support. VAIHCS’ Health Care for Homeless Veterans (HCHV) program serves as the hub for
a myriad of services, with the central goal of ending homelessness among Veterans.
The HCHV program conducts coordinated outreach to proactively seek out Veterans in
need of assistance and connect homeless and at-risk Veterans with housing solutions,
health care, community employment services, and other required supports. Veterans
who are homeless or at imminent risk of becoming homeless can call or visit their local
VA Medical Center, where homeless coordinators and comprehensive HCHV staff are
ready to help. Veterans and their families may also call the national homeless hotline at
1-877-4AID-VET (1-877-424-3838) to access VA services.
